The provider or family refers the patient, contacts the primary physician for eligibility assessment, and initiates hospice care with a physician’s order.
Our representative will answer questions and provide important information about the patient’s care needs and our services during the consultation.
The assessment visit initiates the patient’s transition to hospice by gathering healthcare information, determining eligibility, and creating a personalized care plan.
After document completion, the patient is admitted for hospice care, receiving comprehensive support while you prioritize time with your loved one.
