Shiloh Hospice has been established by providers with more than a decade of practice in health care related settings, in order to provide care, comfort, and support for end-of-life patients and their families. At the heart of Shiloh’s ethos is our aim to bridge the gaps in accessibility of quality care for hospice and palliative care. Our excellent team of interdisciplinary professionals, including doctors, nurses, social workers, chaplains, bereavement co-ordinators, hospice aids, and volunteers, are passionate about sharing in our aim to be the fountain of comfort and support for end-of-life patients. We are proud to deliver patient centred palliative and hospice care to the communities of Home Wood. The provision of our high-quality service to our end-of-life patients and the support their families extends across Cook, Will, and DuPage counties.
Shiloh’s expert team strives to develop individualized plan of care which are sensitive to the requirements and the wishes of every patient. We always act in the best interest of our patients, working with them and their families to develop and deliver patient centred plan of care in line with their needs. We have a wealth of experience in symptom control and pain management, which enables us to limit the amount of pain or discomfort experienced by patients as a result of terminal illness. The Shiloh team specializes in providing hospice and palliative care to patients wherever they call home in, or close to, Home Wood. These settings include private residences and nursing homes, to hospitals or any other location preferred by the patient and their family. We set out to enable our service users to maintain their dignity, autonomy, and preferences throughout their plan of care. Our physician led care plans are driven by compassion for our patients and their loved ones, and we are always sensitive to the unique care needs present in every case. All care delivered by Shiloh Hospice and Palliative Care is therefore professionally carried out according to the wishes of the patient and their loved ones, to meet the needs of the communities we serve in and around Home Wood.
